How to apply for the CDCP

Apply online through My Service Canada Account (recommended), Canada.ca, or by calling Service Canada.

Once approved, you'll receive confirmation. Choose digital communications for faster updates.

Before You Book

A few things to confirm first

The CDCP has coverage limitations depending on the service. Fee differences and out-of-pocket costs may apply based on your income and the treatment you need.

Before booking, check your CDCP enrollment status and renewal date. Coverage must be active on the day of your visit for us to bill the plan directly.

Bring your CDCP confirmation letter, SIN, and photo ID to your first visit. This lets us verify coverage and process billing without delay.
